Online Registration

1. Retired and serving Armed Forces Personnel are required to begin the registration process by filling the online registration form.

2. Complete the on-line registration form and upload relevant documents such as:

•Military ID card (retired or serving)
•Birth certificate(s) of eligible dependants issued by National Population Commission.
•Any other relevant document(s).

Spouse of deceased retired personnel are to upload:
• Letter of condolence from the last unit of the deceased.
• Death certificate.
• Letter of identification of Next of Kin.
• Any other relevant document(s).

Please note that any false information will render the enrollee’s registration invalid.

3. Print your confirmation slip. This slip serves as a means of identification for accessing healthcare services pending the receipt of the permanent ID card.

4. Enrollees can access health care in their chosen Primary Health Care facilities six (6) weeks after the completion of the on-line registration.
